    Nonprofessional Overage Payment

    • Submit your summary Box Office Statement here only if you have A FINAL PAYMENT DUE. If you do not have a Final Payment due, use the Nonprofessional Box Office Statement page.

      Important note: After adding the form to your cart you must complete checkout to submit the summary Box Office Statement and Final Payment. Payment may be made by credit card, PayPal, bank transfer, check, money order, or cashier’s check in US dollars only. You may also choose purchase order as the payment method in order to be sent an invoice to submit to your accounting department for payment.
